| The Surrey Self-Harm Protocol | 
Where hospital care is
Children who have been admitted to a paediatric ward following an episode of self-harm should have:
- An age-appropriate assessment by a mental health professional as soon as possible after arrival to complete a psychosocial assessment in line with the NICE Guidance on Self-harm assessment, management and preventing recurrence.
- A joint daily review by both the paediatric team and children and young people's mental health
- Daily access to their family members or carers, if appropriate.
- Regular multidisciplinary meetings between the general paediatric team and mental health services.
Before discharge ensure that a discharge planning meeting is conducted to include:
- All appropriate agencies and people with arrangements for aftercare specified, including clear written communication with the primary care .
- A plan for further management, drawn up with all appropriate agencies and people.
- Clarity around how best to support the child in their education setting. This should be shared with the School Designated Safeguarding Lead if consent to share has been obtained, or if the Crisis Team consider that there may be a risk to the child or others if the information is not shared.
Valuing mental health equally with physical health is a key priority in paediatric settings, and each acute trust should have a named Mental Health Champion who is a senior clinician.
Emerge Advocacy and east to west provide support for children who are in hospital because they’ve self-harmed, taken an overdose or are feeling suicidal. Staff and volunteers are specifically trained to support young people in Emergency Departments using a youth work approach and provide follow up care in the community after discharge. Emerge Advocacy operate within East Surrey Hospital, Epsom General Hospital, Frimley Park Hospital and Royal Surrey County Hospital. The east to west project operates within Ashford and St Peter’s Hospital.

Acute hospital trusts in Surrey should follow their internal trust pathway for self-harm and contact the Named Professionals within the trust for advice as required.

Where professional dissent occurs at a Child Protection Conference (CP Conference), the Independent CP Conference Chair will make a referral to SSCB Child Protection Dissents (CP Dissents) Group for review. CP Dissents Group is a multi-agency audit group which reviews the conference reports, considers the nature of the dissent and evaluates the decision of the Chair.
The group meets on a monthly basis. The audit enables review of the individual agency’s contribution to the CP Meeting and the professional decision making. Feedback and recommendations are made to the Chair and dissenting professional this includes the identification of key issues, themes and if appropriate recommends where changes are required to policies and procedures.
Where a dissent is upheld feedback is provided to the dissenting professional on the next steps and the decision making referred back to Surrey Children’s Service to investigate. The report and recommendations from this investigation are reviewed by the Head of Safeguarding and actions identified to hold a further Strategy Meeting.
CP Conference Dissents reports into the SSCB Quality Assurance and Evaluation Sub Group as well as the SSCB Operations Group.'

In any situation where it becomes apparent that professionals disagree about the need or otherwise for Child Protection Plan, the relevant professionals, where possible, will discuss this before the conference.
When a conference does not reach a unanimous decision, the chair offers professionals the chance to debrief after the conference.
Where there is a strong difference of opinion, the chair would offer to meet with colleagues dissenting and a manager to explore the differences of opinion and seek a resolution.
Only where this discussion leaves professionals with major unease that they feel the conference decision MUST be revisited should they appeal to external parties to take an independent view. In this case, a request will be made to the Head of Safeguarding in Children's Services to convene a multi-agency group to which the Conference Chair and dissenting professionals will be invited in order to explore the disagreement further.' | 24-08-2016 |
